Integrated Behavioral and Physical Health Care
for Children, Adolescents, and their Families
Vision Statement
We envision a mental health, substance abuse and physical health care
system in our community that is culturally sensitive, coordinated, and
holistic in its approach and is child and family driven. The continuum of
care goes from prevention to intervention with services that are
individualized, seamless, supportive, of excellent quality, accessible
to those that experience barriers to care and measurable. Primary care
is fundamental to our integrated system but care goes beyond the
physician’s offices to places where children live (schools, daycares,
afterschool activities). PCPs collaborate and communicate easily with
individuals, families, caretakers, team members, including other
professionals, maintain confidentiality, learn from their colleagues, and
together practice as a coordinated system. Substance abuse, physical and
mental health are intimately linked in this system to decrease
fragmentation, increase collaboration and most importantly be responsive
to the needs and desires of the children, adolescents, and families that
are served.
